Dementia Care

We believe that with the right care approach, your loved one can live a fulfilled life even in the face of memory loss. Let our team who are trained in Gentle Persuasive Approaches (GPA) in dementia help you create a personalized care plan that delivers stability, support and compassion.

Respite Care

In-home respite care gives the gift of time to family caregivers when they need it most. Whether on a regular weekly schedule or for special events as needed, a well-designed respite care program allows family caregivers an opportunity to recharge physically and emotionally, ensuring they will be able to care for their aging loved one when they need them most. Our Care Professionals provide peace of mind by ensuring the aging adult is cared for while the family caregiver focuses on self-care

Companion and Support

Our compassionate, trustworthy and  personal support workers are available to assist with dressing, eating, grooming and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.

Hospice Care

When a loved one chooses hospice care, you should be able to focus on being a loving spouse, child or grandchild without having to wear the badge of “Caregiver”. Our in-home Hospice Care Support Services can do just that and allow you the time to focus on what matters most, spending precious moments with the one you love

Hospital-home support/Live-in Care

If your loved one had a recent fall or hospitalization or is living with a debilitating condition like a stroke or dementia, they may need around-the-clock care, which can be challenging for family caregivers. Whether in the overnight hours or during the day, our team of Care Professionals support family caregivers to provide essential care 24-hours a day.

Why CAC Home Health

We are available 24/7-  24 hours a day, seven days a week, 365 days a year. We can provide services up to 24 hours a day and there is always on-call  manager   who can answer your questions and address any concerns after hours. The on-call manager also provides around-the-clock clinical support for your healthcare team while they are providing care in the home.

We are local. CAC home health is local. Our aim is to make a positive impact on the community and the people in our care.

We are selective. All nurses and PSW’s who visit your home go through a rigorous interview and screening process, as well as a background checks and license verification.  Our caregivers are regularly supervised and evaluated to insure that you or your loved one is receiving the care and attention they deserve.

We are flexible. If you are unsatisfied with a particular caregiver, all it takes is a phone call. We can custom match another caregiver to a client at any time.

We are protected. We carry all of the necessary liability coverage so that all of our clients and their families are fully protected.

We are consistent and reliable. Once a care team has been designated to you, we aim to maintain that team for your care so that you see familiar faces and know who to expect in your home. If your regular carer cannot attend the shift, we keep you informed and do our best to find a replacement staff member to perform the service at a suitable time for you.

COVID-19

Your safety comes first!  At CAC Home Health  we follow all CDC guidelines to do our part in preventing the spread of COVID-19.  Our employee’s go through Covid-19 screening prior to the start of each shift.  Staff are verbally screened for any signs/symptoms that may be related to Covid-19, this includes:  Fever, Cough, Shortness of Breath, Loss of Appetite, Rash just to name a few. Our Staff are required to wear face coverings and follow all required infection control procedures.

Should Staff or Care Recipient exhibit symptoms, CAC Home Health requires Covid-19 testing to be completed.  Staff will not be allowed to return to work until testing indicates a negative result for Covid-19.
